New Patient Registration

If you live within the practice boundary of Burntisland, Kinghorn, Aberdouryou are welcome to register with the practice.

When you request registration you will be given the option of two appointments, one with the GP ( to discuss any medication etc.) and one with the nurse for a general health check. 

You will be asked to sign a registration form and will be given a questionnaire  to fill in with details of your previous medical history. Please bring two forms of ID ideally passport, driving licence, birth certificate or utility bill.

Appointments should be made by contacting the receptionist either by telephone or in person.

When you arrive for your appointment you can check in at the reception desk or use the self check in machine.

If you are registering a new baby, you should bring the form issued from the Registrar’s office. 

Medical treatment is available from the date of registration. Please contact reception for further information.

Changes of Personal Details

If your name, address or telephone number change, please let us know as soon as possible.  It is important that we have your correct details so that we can contact you if necessary and deal with your health care matters efficiently.
